Cranial Hair Prosthesis

Cranial prosthesis is medical terminology used to properly document and treat hair loss through the use of hair replacement or medical wigs.  At Salon Meamoure, we have done our research and are now offering cranial prosthesis to our clients/patients that have undergone medical conditions leading to hair loss.  Such medical conditions include but are not limited to:  chemotherapy,  pregnancy, thyroid disorders, and anemia. Others include autoimmune diseases, polycystic ovary syndrome (PCOS), and skin conditions such as psoriasis and seborrheic dermatitis.  We encourage you to visit your doctor before purchasing a wig to ensure that your wig is first prescribed and then will be covered under your existing health care plan.  

 

The Process:

1. Contact us for a custom estimate at meamourestyles@gmail.comThen ask your insurance provider how much they will reimburse you for your medical hair prosthesis, and how long it will take to get reimbursed. 

Sample Estimate: P.S. Don't be frightened by the sample estimate as colour, and additional services are listed as a sample.   

2. Purchase a Wig

Schedule a free consultation at our salon (open 5 days a week). We will listen to your hair loss concerns, and answer all your questions. We will measure your head to ensure an accurate fitting.  Each wig is styled by our talented and compassionate wig stylist. Our goal is help you look and feel like yourself again. Then when you are ready, you can purchase a wig. 

3. File a Claim

Download and send Claim Form #1500 to your insurance company (contact your provider if the form is not listed below). Other items you will need include:

  1. Our Federal Tax ID: #47-2684622

  2. Procedure Code from your insurance provider. Commonly used procedure codes:   S8095 and A9282

  3. Receipt paid in full as proof of payment

  4. Rx with your diagnosis code from your doctor (It must be written for a medical or cranial hair prosthesis)
